From and after the effective date of the ordinance codified in this chapter an eligible head of household, whether that individual lives within or outside the city limits, may apply for a senior citizen or disabled citizen rate discount for water, sewer, or storm drainage, as applicable. All rate discounts are subject to the following terms and conditions:

A. Only one such discount for each utility service is granted per dwelling unit.

B. The applicant shall certify under penalty of perjury on a form presented to the city clerk that the applicant at the time of the claim of the discount is no less than sixty-five years of age, or is permanently disabled and is the head of household of the property claiming the discount, and that the applicant(s) for the previous calendar year had a total income from all sources of less than the maximum allowed for very low income as allowed by the Department of Housing and Urban Development for the previous year.
